Most patients are seen by appointment. In most cases, referrals are required from your physician. Please arrive at least 15 minutes prior to your appointment time in order to complete any necessary paperwork. Please bring your insurance information with you.

We make every effort to stay on time, so your wait will be minimal. On rare occasions, emergencies may arise that will put us behind schedule. At such times, your patience is appreciated. If you are unable to keep a scheduled appointment, we ask that you call us to cancel or reschedule 24 hours in advance.
